| Metropolitan Area Agency on Aging (MAAA) |
A fresh annual report approach was needed for current stakeholders and new constituents. Custom Health Consultants created a fresh appearance for the 2007 MAAA Annual Report. The new attractive, user-friendly publication features the agency's recognizable design palette and compelling client stories on the impact of agency care. |

| SouthEast Seniors Agenda for Independent Lliving (SAIL) |
An on-line curriculum on organizational sustainability was needed for senior service providers in Minnesota. SAIL contracted Custom Health Consultants to design three on-line modules. We conducted extensive interviews and research on financial sustainability, strategic positioning, and community engagement to determine critical content. The three modules were written, edited, produced on-line and are now used by professionals offering transportation, nutrition, adult day care, assisted living, and other older adult care. The on-line curriculum offers best practices and core principles for executive directors, financial officers, managers, board members, and others in senior care adults leadership positions. |

To meet grant writing needs, the International Institute asked Custom Health Consultants to help meet their ongoing grant deadlines to fund refugee student advancement in health careers. |
| National Hospice & Palliative Care Organization (NHPCO) |
The National Hospice and Palliative Care Organization needed a national toolbox that would contain best practices and case studies of multicultural hospice care. Custom Health Consultants was asked to create an infrastructure and copy for the publication. As a participant on a national advisory board on multicultural hospice & palliative care, we guided the creation of the toolbox from copy writing to reviewing design elements. Today, the Access & Inclusion Toolbox is available to 4,000 hospices throughout the United States. It is a national guide for hospice & palliative care physicians, nurses, social workers, chaplains, music therapists and others in end-of-life care.
