  NATIONAL HOSPICE & PALLIATIVE CARE ORGANIZATION (NHPCO)  

The National Hospice & Palliative Care Organization (NHPCO) identified a national need for multicultural end-of-life training among its 4,000 member organizations.  Custom Health Consultants was invited to help develop a national toolbox.  This included writing chapter content, forming a structure and editing many of the chapters.

This resource is now used by physicians, nurses, social workers, chaplains, music therapists, executive directors, and hospice board members.

NHPCO later developed an on-line course to increase access & inclusion in end-of-life care.   The breadth of inclusion includes race, ethnicity, religion & spirituality, culture, gender, physical abilities and beliefs.  Barbara Greene, MPH, Custom Health Consultants, is a frequent instructor and facilitator.

 

  CAPI  

What role do refugees & immigrants play in the leadership of our cities and state?  Custom Health Consultants was invited to help CAPI develop a high-level summit that included resettlement agencies, foundations, health care, education, and other organizations.

The "sold out" event featured an exemplary panel of refugee & immigrant leaders - including a national writer, foundation executive director, community activist, international photographer, state legislator, and policy maker.

Several important outcomes occurred:  CAPI increased its capacity to reach into the broader refugee & immigrant community as a catalyst for change.  On a larger level, relationships between immigrant communities were developed and strengthened.

  HOSPICE MINNESOTA

Over a four year project period, Custom Health Consultants helped Hospice Minnesota to train hundreds of hospice care professionals in Minnesota in culturally respectful end-of-life care involving multicultural communities.


New relationships with multicultural community leaders were forged.  New community partnerships were developed between multicultural organizations and hospice professionals.


Custom Health Consultants created new bilingual/bicultural hospice materials for the Hmong, Somali and Latino communities, as well as for hospice professionals.


An extensive web-based resource was developed for Minnesota health care professionals on multicultural resources, agencies and training in end-of-life care.

  METROPLITAN AREA AGENCY ON AGING (MAAA)

Custom Health Consultants helped MAAA plan a statewide continuing education conference on aging & disabilities. The conference featured plenary and concurrent sessions and a large exhibitor center. A new conference website publicized speakers, exhibitors and participant handouts.


A highly successful White House Conference on Aging was planned for professionals in the field of aging. Outcomes included action items to national delegates on the health and well-being of older Americans.


Grants were written to support metro-wide services for vulnerable older adults.

  CITY OF MINNEAPOLIS HEALTH & FAMILY SUPPORT

Custom Health Consultants helped plan and implement a city-wide health & fitness celebration for community members participating in weight loss outcomes. Sponsors and pro-bono items were distributed to winning teams with lifestyle changes and fitness outcomes.


Community health grants were written to reduce health disparities among multicultural communities. Funding was successfully secured to increase health promotion/disease prevention in diverse urban communities.
